Momentum Brazilian Jiu-Jitsu - Waiver of Claims and Liability
All students must sign this waiver of claims and liability. By signing this waiver of claims and liability the student, parent or guardian acknowledges that they have read this agreement and will abide by all rules of Momentum BJJ.
Signing this waiver also means that the undersigned/student releases Momentum BJJ, Julio Galarza or Momentum BJJ's instructors from any liability in case of injury or death from the instruction at Momentum BJJ.
In consideration of being allowed to attend this Brazilian Jiu-Jitsu/Grappling/Self-Defense/Kids Class/Boxing program, in either group or private classes, the undersigned/student acknowledges that Brazilian Jiu-Jitsu/Grappling/Self-Defense/Kids
Classes/Boxing are physical activities and that participation may result in serious injury, including death and or other damage, due not only to the undersigned/students actions, inaction or negligence, but also to that of others associated with and or present at these or other classes. Furthermore, the undersigned/student acknowledges that there may be risks not known to them and or not reasonably foreseeable at this time. The undersigned/student personally, totally and completely assume all risks involved or in any way related to their participation in this Brazilian Jiu-Jitsu/Grappling/Self- Defense/Kids Class/Boxing program whether known or unknown to them at the present time. Participation in this Brazilian Jiu-Jitsu/Grappling/Self-Defense/Kids Class/Boxing program is voluntary and entirely of my own free will.
The undersigned/student hereby represents and certifies that they are in good physical condition, and have no diseases, injuries or other physical or mental conditions that would impair their ability to participate in the Brazilian Jiu-Jitsu/Grappling/Self-Defense/Kids Class/Boxing program. The undersigned/student and their heirs, executors and successors hereby waive claim of accidental and or negligent damages or injury against Momentum BJJ, Julio Galarza and Momentum BJJ's instructors and any physical location where a Momentum BJJ class is taught.
The undersigned/student also certifies that they are/will be covered by a medical insurance plan and will assume responsibility for all bills/costs associated with injuries sustained while attending or participating in a Momentum BJJ class/seminar/workshop/camp.
